What to Bring to Your Appointment

  • Your insurance card
  • Physician referral forms, if required by insurance
  • List of prescriptions and/or over-the-counter medication, including dose and frequency. Also, bring your pharmacy address and telephone number
  • Information about your medical and surgical history
  • Relevant medical records

Insurance Information

We participate in most managed health care plans in Central Florida, including Medicare and Medicaid.

HMO patients are expected to notify our office if there are any changes in medical health insurance, name, address, telephone number, and/or any other pertinent personal and/or medical information.

Patients are responsible for obtaining the appropriate referral from their PCP prior to being evaluated by our physicians.

Check with our physicians prior to getting the allergy injection if you are sick.
